Collaborative Documentation Pilot
Transforming client engagement and decreasing staff administrative burden
Purpose
Collaborative Documentation (CD) is an evidence-informed clinical practice that promotes client-facing staff documenting with clients present, engaging them in creating the content and incorporating their perspective into the record.
Implementing CD as a pilot allows an organization to engage selected staff in the design and development of effective implementation strategies that will help with full adoption throughout the organization. These early adopters will become the coaches and mentors for their peers.
Process
Planning meeting with the organizational leadership/project team
Training to motivate staff to embrace CD and start implementation immediately.
Post training coaching and Q&A sessions
Provision of pilot implementation tools, client and staff surveys, and training handouts
Timeframe: 1-3 months
Outcomes:
● Clinicians save significant time -- an average of 200-250 hours per year-
reducing burnout and achieving a better work-life balance.
● Clients report 95% approval ratings, greater medication adherence, and
demonstrate improved engagement through fewer no-shows.
● Organizations benefit from improved documentation accuracy, compliance, and timeliness while reducing risk through enhanced care coordination
