Transforming client engagement and decreasing staff administrative burden


Purpose

Collaborative Documentation (CD) is an evidence-informed clinical practice that promotes client-facing staff documenting with clients present, engaging them in creating the content and incorporating their perspective into the record. 

The Collaborative Documentation Train the Trainer Module is designed to support CD sustainability within the organization through internal training of all staff as part of the new hire process and maintaining fidelity to the model through coaching, quarterly monitoring, refresher courses, and annual update requirements.    

Process

  • Kick off meeting to review Collaborative Documentation (CD) and start the change process with the Rapid Cycle Change Team (RCCT) assigned to implement CD.

  • RCCT meetings

  • Training for up to 50 clinical staff members including organization’s identified trainers

  • rain the Trainer for up to 20 staff to review CD Materials in depth, and discuss training strategies, implementation challenges and rapid cycle change management.

  • Training for Medical Providers (optional)

  • Trainer meetings post implementation to review progress and address barriers.

  • Essential Skills Training to support successful implementation of CD as determined by the organization and consultant

  • Creation of customized training materials.

  • Provision of implementation tools and handouts, and client and staff surveys.

Timeframe: 3-4 months

Outcomes:

Clinicians save significant time -- an average of 200-250 hours per year-
reducing burnout and achieving a better work-life balance.

demonstrate improved engagement through fewer no-shows.
