Purpose
Clients experience better outcomes when care is coordinated within the organization, with primary care providers and with social supports. This aligns with Value Based Care expectations that healthcare systems focus on the Right Person providing the Right Care at the Right Time, Frequency and Duration. Behavioral Health organizations need to implement strategies to support internal and external collaboration. MTM’s Integrated Care Module will provide an objective review of the current level of care integration within the organization, and with primary care and other critical stakeholders.
Process
Planning meeting with project team to review written plan of roles, responsibilities, and time frames related to the project
Analysis of current level of integration and barriers to implementation
Creation of a plan for care integration throughout the system of care including communication, implementation and maintenance strategies for successful implementation
Timeframe: 4 months
Outcomes:
● Increased internal and external service delivery integration, practice standardization, improved client outcomes, successful implementation of critical CCBHC Requirements and significant Risk Reduction.

The plan will align with community, state and federal expectations for collaboration and seamless care.
